While handling an I/O completion for the compare portion of a
COMPARE_AND_WRITE command, it may happen that the
compare_and_write_callback function submits new bio structs while still in
softirq context.
Low level drivers like md raid5 do not expect their make_request call to be
used in softirq context, they call into schedule() and create a deadlocked
system.

This problem appears to be an issue between target_cmd_interrupted() and
compare_and_write_callback(). target_cmd_interrupted() calls the se_cmd's
transport_complete_callback function pointer if the se_cmd is being stopped
or aborted, and CMD_T_ABORTED was set on the se_cmd.
When calling compare_and_write_callback(), the success parameter was set to
false. target_cmd_interrupted() seems to expect this means the callback
will do cleanup that does not require a process context. But
compare_and_write_callback() ignores the parameter if there was I/O done
for the compare part of COMPARE_AND_WRITE.
Since there was data, the function continued on, passed the compare, and
issued a write while ignoring the value of the success parameter. The
submit of a bio for the write portion of the COMPARE_AND_WRITE then causes
schedule to be unsafely called from the softirq context.
Fix the bug in compare_and_write_callback by jumping to the out label if
success == "false", after checking if we have been called by
transport_generic_request_failure(); The command is being aborted or
stopped so there is no need to submit the write bio for the write part of
the COMPARE_AND_WRITE command.

This patch removes XDWRITEREAD support because it never fully worked when
it was added in the initial LIO merge and it's been fully broken since 2013
from commit a2890087499a ("target: Add compare_and_write_post() completion
callback fall through").
The two issues above are:
1. XDWRITEREAD support was just never completed when LIO was merged. We
never did the DISABLE WRITE check and so we never write data out.
2. Since the commit above, we never complete the command. After we do the
XOR, we return from xdreadwrite_callback and that's it. We never send a
response for the command, so the command will always time out and fail.
Since this has been fully broken for almost nine years this patch just
removes emulated support.

In newer version of the SBC specs, we have a NDOB bit that indicates there
is no data buffer that gets written out. If this bit is set using commands
like "sg_write_same --ndob" we will crash in target_core_iblock/file's
execute_write_same handlers when we go to access the se_cmd->t_data_sg
because its NULL.
This patch adds a check for the NDOB bit in the common WRITE SAME code
because we don't support it. And, it adds a check for zero SG elements in
each handler in case the initiator tries to send a normal WRITE SAME with
no data buffer.

SBC-4 r15 5.3 COMPARE AND WRITE command states:
if the compare operation does not indicate a match, then terminate the
command with CHECK CONDITION status with the sense key set to
MISCOMPARE and the additional sense code set to MISCOMPARE DURING
VERIFY OPERATION. In the sense data (see 4.18 and SPC-5) the offset
from the start of the Data-Out Buffer to the first byte of data that
was not equal shall be reported in the INFORMATION field.
This change implements the missing logic to report the miscompare offset in
the sense data INFORMATION field. As an optimization, byte-by-byte
miscompare offset calculation is only performed after memcmp() mismatch.

cmd.bad_sector currently gets packed into the sense INFORMATION field for
TCM_LOGICAL_BLOCK_{GUARD,APP_TAG,REF_TAG}_CHECK_FAILED errors, which carry
an .add_sector_info flag in the sense_detail_table to ensure this.
In preparation for propagating a byte offset on COMPARE AND WRITE
TCM_MISCOMPARE_VERIFY error, rename cmd.bad_sector to cmd.sense_info and
sense_detail.add_sector_info to sense_detail.add_sense_info so that it
better reflects the sense INFORMATION field destination.

COMPARE AND WRITE command execution starts with a call of
sbc_compare_and_write(). That function locks the caw_sem member in the
backend device data structure and submits a read request to the backend
driver. Upon successful completion of the read compare_and_write_callback()
gets called. That last function compares the data that has been read. If it
matches transport_complete_callback is set to compare_and_write_post and a
write request is submitted. compare_and_write_post() submits a write request
to the backend driver.
XDWRITEREAD command execution starts with sbc_execute_rw() submitting a
read to the backend device. Upon successful completion of the read the
xdreadwrite_callback() gets called. That function xors the data that has
been read with the data in the data-out buffer and stores the result in
the data-in buffer.
Call transport_complete_callback() not only if COMPARE AND WRITE fails but
also if XDWRITEREAD fails. This makes the code more systematic. Make sure
that the callback functions handle (cmd, false, NULL) argument triples fine.

The purpose of sg_alloc_table() is to allocate and initialize an
sg-list. Use that function instead of open-coding it. This patch will
make it easier to share code for caching sg-list allocations between the
SCSI and NVMe target cores.

If a length of a range is zero, it means there is nothing to unmap
and we can skip this range.
Here is one more reason, why we have to skip such ranges. An unmap
callback calls file_operations->fallocate(), but the man page for the
fallocate syscall says that fallocate(fd, mode, offset, let) returns
EINVAL, if len is zero. It means that file_operations->fallocate() isn't
obligated to handle zero ranges too.

In struct se_dev_attrib, there is a field emulate_caw exposed
as a /sys/kernel/config/target/core/$HBA/$DEV/attrib/.
If this field is set zero, it means the corresponding struct se_device
does not support the scsi cmd COMPARE_AND_WRITE
In function sbc_parse_cdb(), go ahead and reject scsi COMPARE_AND_WRITE
if emulate_caw is not set, because it has been explicitly disabled
from user-space.

This patch broke existing behaviour for WRITE_VERIFY because
it dropped the original SCF_SCSI_DATA_CDB assignment for
bytchk = 0 so target_cmd_size_check() no longer rejected
this case, allowing an overflow case to trigger an OOPs
in iscsi-target.
Since the short term and long term fixes are still being
discussed, revert it for now since it's late in the merge
window and try again in v4.13-rc1.

Following the bugfix for handling non SAM_STAT_GOOD COMPARE_AND_WRITE
status during COMMIT phase in commit 9b2792c3da1, the same bug exists
for the READ phase as well.
This would manifest first as a lost SCSI response, and eventual
hung task during fabric driver logout or re-login, as existing
shutdown logic waited for the COMPARE_AND_WRITE se_cmd->cmd_kref
to reach zero.
To address this bug, compare_and_write_callback() has been changed
to set post_ret = 1 and return TCM_LOGICAL_UNIT_COMMUNICATION_FAILURE
as necessary to signal failure status.

* A multiplication for the size determination of a memory allocation
indicated that an array data structure should be processed.
Thus use the corresponding function "kmalloc_array".
This issue was detected by using the Coccinelle software.
* Replace the specification of a data structure by a pointer dereference
to make the corresponding size determination a bit safer according to
the Linux coding style convention.

Use the value of the BYTCHK field to determine the size of the
Data-Out buffer. For VERIFY, honor the VRPROTECT, DPO and FUA
fields. This patch avoids that LIO complains about a mismatch
between the expected transfer length and the SCSI CDB length
if the value of the BYTCHK field is 0.

As reported by Max, the Windows 2008 R2 chkdsk utility expects
VERIFY_16 to be supported, and does not handle the returned
CHECK_CONDITION properly, resulting in an infinite loop.
The kernel will log huge amounts of this error:
kernel: TARGET_CORE[iSCSI]: Unsupported SCSI Opcode 0x8f, sending
CHECK_CONDITION.

The patch that reworks task management function handling guarantees
that target_remove_from_state_list() is always called with CMD_T_BUSY
cleared. Since that function is the only function that tests that flag
this means that that flag is now superfluous. Hence remove that flag.

This patch addresses a long standing bug where the commit phase
of COMPARE_AND_WRITE would result in a se_cmd->cmd_kref reference
leak if se_cmd->scsi_status returned non SAM_STAT_GOOD.
This would manifest first as a lost SCSI response, and eventual
hung task during fabric driver logout or re-login, as existing
shutdown logic waited for the COMPARE_AND_WRITE se_cmd->cmd_kref
to reach zero.
To address this bug, compare_and_write_post() has been changed
to drop the incorrect !cmd->scsi_status conditional that was
preventing *post_ret = 1 for being set during non SAM_STAT_GOOD
status.
This patch has been tested with SAM_STAT_CHECK_CONDITION status
from normal target_complete_cmd() callback path, as well as the
incoming __target_execute_cmd() submission failure path when
se_cmd->execute_cmd() returns non zero status.

Remove superfluous #include directives from the include/target/*.h
files. Add missing #include directives to other *.h and *.c files.
Use forward declarations for structures where possible. This
change reduces the build time for make M=drivers/target on my
laptop from 27.1s to 18.7s or by about 30%.

If a command with a Simple task attribute is failed due to a Unit
Attention, then a subsequent command with an Ordered task attribute
will hang forever. The reason for this is that the Unit Attention
status is checked for in target_setup_cmd_from_cdb, before the call
to target_execute_cmd, which calls target_handle_task_attr, which
in turn increments dev->simple_cmds.
However, transport_generic_request_failure still calls
transport_complete_task_attr, which will decrement dev->simple_cmds.
In this case, simple_cmds is now -1. So when a command with the
Ordered task attribute is sent, target_handle_task_attr sees that
dev->simple_cmds is not 0, so it decides it can't execute the
command until all the (nonexistent) Simple commands have completed.

This change sets the LBPRZ flag in EVPD page b2h and READ CAPACITY (16)
based on a new unmap_zeroes_data device attribute. This flag is set
automatically for iblock based on underlying block device queue's
discard_zeroes_data flag.

target_core_sbc's compare_and_write functionality suffers from taking
data at the wrong memory location when writing a CAW request to disk
when a SGL offset is non-zero.
This can happen with loopback and vhost-scsi fabric drivers when
SCF_PASSTHROUGH_SG_TO_MEM_NOALLOC is used to map existing user-space
SGL memory into COMPARE_AND_WRITE READ/WRITE payload buffers.
Given the following sample LIO subtopology,

Rather than writing all-zeroes as instructed with the -D file, it
corrupts the data in the sector by splicing some of the original
bytes in. The page of the first entry of cmd->t_data_sg includes the
CDB, and sg->offset is set to a position past the CDB. I presume that
sg->offset is also the right choice to use for subsequent sglist
members.

This patch addresses a race + use after free where the first
stage of COMPARE_AND_WRITE in compare_and_write_callback()
is rescheduled after the backend sends the secondary WRITE,
resulting in second stage compare_and_write_post() callback
completing in target_complete_ok_work() before the first
can return.
Because current code depends on checking se_cmd->se_cmd_flags
after return from se_cmd->transport_complete_callback(),
this results in first stage having SCF_COMPARE_AND_WRITE_POST
set, which incorrectly falls through into second stage CAW
processing code, eventually triggering a NULL pointer
dereference due to use after free.
To address this bug, pass in a new *post_ret parameter into
se_cmd->transport_complete_callback(), and depend upon this
value instead of ->se_cmd_flags to determine when to return
or fall through into ->queue_status() code for CAW.

The fabric API usage for se_node_acl allocation is now within
target-core code, dropping the external API callers for all fabric
drivers tree-wide.
There is a new conversion to RCU hlists for se_node_acl and
se_portal_group LUN mappings, that turns fast-past LUN lookup into a
completely lockless code-path. It also removes the original
hard-coded limitation of 256 LUNs per fabric endpoint.
The configfs attributes for backends can now be shared between core

"This is the usual grab bag of driver updates (lpfc, hpsa,

There is also one new driver: the Cisco snic. The advansys driver has

Only include SCSI initiator header files in target code that needs
these header files, namely the SCSI pass-through code and the tcm_loop
driver. Change SCSI_SENSE_BUFFERSIZE into TRANSPORT_SENSE_BUFFER in
target code because the former is intended for initiator code and the
latter for target code. With this patch the only initiator include
directives in target code that remain are as follows:

sbc_dif_generate() and sbc_dif_verify() currently assume that each
SG element for data transfer memory doesn't straddle the block size
boundary.
However, when using SG_IO ioctl, we can choose the data transfer
memory which doesn't satisfy that alignment requirement.
In order to handle such cases correctly, this change inverts the outer
loop to iterate data transfer memory and the inner loop to iterate
protection information and enables to calculate CRC for a block which
straddles multiple SG elements.

sbc_dif_copy_prot()
In sbc_dif_copy_prot(), the addresses passed to kunmap_atomic() are
inconsistent with the addresses which are mapped by kmap_atomic().
That could be problematic if an SG element has its length larger than
PAGE_SIZE as kunmap_atomic() will attempt to unmap different page.

Drivers may override the WCE flag, in which case the DPOFUA flag in
MODE SENSE might differ from the check used to reject invalid FUA
bits in sbc_check_dpofua. Also now that we reject invalid FUA
bits early there is no need to duplicate the same buggy check
down in the fileio code.
As the DPOFUA flag controls th support for FUA bits on read and
write commands as well as DPO key off all the checks off a single
helper, and deprecate the emulate_dpo and emulate_fua_read attributs.
This fixes various failures in the libiscsi testsuite.
Personally I'd prefer to also remove the emulate_fua_write attribute
as there is no good reason to disable it, but I'll leave that for
a separate discussion.

Instead of providing DIF verify routines for read/write
that are almost identical and conditionally copy protection
information, just let the caller do the right thing.
Have a single sbc_dif_verify that handles an sgl (that
does NOT copy any data) and a protection information copy
routine used by rd_mcp and fileio backend.
In the WRITE case, call sbc_dif_verify with cmd->t_prot_sg
and then do the copy from it to local sgl (assuming the verify
succeeded of course). In the READ case, call sbc_dif_verify
with the local sgl and if it succeeds, copy it to t_prot_sg (or
not if we are stripping it).
