Pull SCSI updates from James Bottomley:
"Updates to the usual drivers (qla2xxx, lpfc, ufs, hisi_sas, mpi3mr,
mpt3sas, target). The biggest change (from my biased viewpoint) being
that the mpi3mr now attached to the SAS transport class, making it the
first fusion type device to do so.
Beyond the usual bug fixing and security class reworks, there aren't a
huge number of core changes"

Everything is just converted to returning RQ_END_IO_NONE, and there
should be no functional changes with this patch.
In preparation for allowing the end_io handler to pass ownership back
to the block layer, rather than retain ownership of the request.

Let the caller set it together with the end_io_data instead of passing
a pointless argument. Note the the target code did in fact already
set it and then just overrode it again by calling blk_execute_rq_nowait.

With tape devices, the SCF_TREAT_READ_AS_NORMAL flag is used by the target
subsystem to mark commands which have both data to return as well as sense
data. But with pscsi, SCF_TREAT_READ_AS_NORMAL can be set even if there is
no data to return. The SCF_TREAT_READ_AS_NORMAL flag causes the target core
to call iscsit data-in callbacks even if there is no data, which iscsit
does not support. This results in iscsit going into an error state
requiring recovery and being unable to complete the command to the
initiator.
This issue can be resolved by fixing pscsi to only set
SCF_TREAT_READ_AS_NORMAL if there is valid data to return alongside the
sense data.

Remove the magic autofree semantics and require the callers to explicitly
call bio_init to initialize the bio.
This allows bio_free to catch accidental bio_put calls on bio_init()ed
bios as well.

Now that each scsi_request is backed by a scsi_cmnd, there is no need to
indirect the CDB storage. Change all submitters of SCSI passthrough
requests to store the CDB information directly in the scsi_cmnd, and while
doing so allocate the full 32 bytes that cover all Linux supported SCSI
hosts instead of requiring dynamic allocation for > 16 byte CDBs. On
64-bit systems this does not change the size of the scsi_cmnd at all, while
on 32-bit systems it slightly increases it for now, but that increase will
be made up by the removal of the remaining scsi_request fields.

Copy directly from the se_cmd CDB to the one in the scsi_request. This
temporarily limits the pscsi backend to supporting only up to 16 byte CDBs,
but this restriction will be lifted later in this series.

There is no good reason to keep genhd.h separate from the main blkdev.h
header that includes it. So fold the contents of genhd.h into blkdev.h
and remove genhd.h entirely.

Add a new helper that calls blk_get_request and initializes the
scsi_request to avoid the indirect call through ->.initialize_rq_fn.
Note that this makes the pktcdvd driver depend on the SCSI core, but
given that only SCSI devices support SCSI passthrough requests that
is not a functional change.

pscsi_complete_cmd()
The return value of transport_kmap_data_sg() is assigned to the variable
buf:
buf = transport_kmap_data_sg(cmd);
And then it is checked:
if (!buf) {
This indicates that buf can be NULL. However, it is dereferenced in the
following statements:
if (!(buf[3] & 0x80))
buf[3] |= 0x80;
if (!(buf[2] & 0x80))
buf[2] |= 0x80;
To fix these possible null-pointer dereferences, dereference buf and call
transport_kunmap_data_sg() only when buf is not NULL.

With the legacy IDE driver gone drivers now use either REQ_OP_DRV_*
or REQ_OP_SCSI_*, so unify the two concepts of passthrough requests
into a single one.

Instead of overloading the passthrough fast path with the deprecated
block layer bounce buffering let the users that combine an old
undermaintained driver with a highmem system pay the price by always
falling back to copies in that case.

pscsi_map_sg() uses the variable nr_pages as a hint for bio_kmalloc() how
many vector elements to allocate. If nr_pages is < BIO_MAX_PAGES, it will
be reset to 0 after successful allocation of the bio.
If bio_add_pc_page() fails later for whatever reason, pscsi_map_sg() tries
to allocate another bio, passing nr_vecs = 0. This causes bio_add_pc_page()
to fail immediately in the next call. pci_map_sg() continues to allocate
zero-length bios until memory is exhausted and the kernel crashes with
OOM. This can be easily observed by exporting a SATA DVD drive via pscsi.
The target crashes as soon as the client tries to access the DVD LUN. In
the case I analyzed, bio_add_pc_page() would fail because the DVD device's
max_sectors_kb (128) was exceeded.
Avoid this by simply not resetting nr_pages to 0 after allocating the
bio. This way, the client receives an I/O error when it tries to send
requests exceeding the devices max_sectors_kb, and eventually gets it
right. The client must still limit max_sectors_kb e.g. by an udev rule if
(like in my case) the driver doesn't report valid block limits, otherwise
it encounters I/O errors.

It's often inconvenient to use BIO_MAX_PAGES due to min() requiring the
sign to be the same. Introduce bio_max_segs() and change BIO_MAX_PAGES to
be unsigned to make it easier for the users.

The 'q' is not used since commit a1ce35fa4985 ("block: remove dead
elevator code"), also update the comment of the function.

Now that the hd_struct always has a block device attached to it, there is
no need for having two size field that just get out of sync.
Additionally the field in hd_struct did not use proper serialization,
possibly allowing for torn writes. By only using the block_device field
this problem also gets fixed.

pgr_support and alua_support device attributes show the inverted value of
the transport_flags:
* TRANSPORT_FLAG_PASSTHROUGH_PGR
* TRANSPORT_FLAG_PASSTHROUGH_ALUA
These attributes are per device, while the flags are per backend. Rename
the transport_flags in backend/transport to transport_flags_default and use
this value to initialize the new transport_flags field in the se_device
structure.
Now data and attribute both are per se_device.

In preparation for supporting user provided vendor strings, add an extra
byte to the vendor, model and revision arrays in struct t10_wwn. This
ensures that the full INQUIRY data can be carried in the arrays along with
a null-terminator.
Change a number of array readers and writers so that they account for
explicit null-termination:
- The pscsi_set_inquiry_info() and emulate_model_alias_store() codepaths
don't currently explicitly null-terminate; fix this.
- Existing t10_wwn field dumps use for-loops which step over
null-terminators for right-padding.
+ Use printf with width specifiers instead.

When a tape drive is exported via LIO using the pscsi module, a read
that requests more bytes per block than the tape can supply returns an
empty buffer. This is because the pscsi pass-through target module sees
the "ILI" illegal length bit set and thinks there is no reason to return
the data.
This is a long-standing transport issue, since it assumes that no data
need be returned under a check condition, which isn't always the case
for tape.
Add in a check for tape reads with the ILI, EOM, or FM bits set, with a
sense code of NO_SENSE, treating such cases as if the read
succeeded. The layered tape driver then "does the right thing" when it
gets such a response.

When the current page can't be added to bio, one new bio should be
created for adding this page again, instead of ignoring this page.
This patch fixes kernel crash with iscsi target and dvd, as reported by
Wakko.

Commit caa4b02476e3(blk-map: call blk_queue_bounce from blk_rq_append_bio)
moves blk_queue_bounce() into blk_rq_append_bio(), but don't consider
the fact that the bounced bio becomes invisible to caller since the
parameter type is 'struct bio *'. Make it a pointer to a pointer to
a bio, so the caller sees the right bio also after a bounce.

TYPE_ZBC host managed zoned block devices are also block devices
despite the non-standard device type (14h). Handle them similarly to
regular TYPE_DISK devices.

This patch performs the pscsi_transport_complete operations from
pscsi_req_done. It looks like the only difference the transport_complete
callout provides is that it is called under t_state_lock which seems to
only be needed for the SCF_TRANSPORT_TASK_SENSE bit handling. We can
now use transport_copy_sense_to_cmd to handle the se_cmd sense bits, and
we can then drop the code where we have to copy the request info to
the pscsi_plugin_task for transport_complete use.

With this patch free_device is now used to free what is allocated in the
alloc_device callback and destroy_device tears down the resources that are
setup in the configure_device callback.
This patch will be needed in the next patch where tcmu needs
to be able to look up the device in the destroy callback.
