This adds the ethernet driver for Agere et131x devices to
drivers/net/ethernet.
The driver being added has been in the staging tree for some time, and will be
removed from there in a seperate patch. This one merely disables the staging
version to prevent two instances being built.

the prerelease ones used for driver development.

Pull vfs fixes from Al Viro:
"double iput() on failure exit in lustre, racy removal of spliced
dentries from ->s_anon in __d_materialise_dentry() plus a bunch of
assorted RCU pathwalk fixes"
The RCU pathwalk fixes end up fixing a couple of cases where we
incorrectly dropped out of RCU walking, due to incorrect initialization
and testing of the sequence locks in some corner cases. Since dropping
out of RCU walk mode forces the slow locked accesses, those corner cases
slowed down quite dramatically.

According to the documentation sync_fence_create takes ownership of the point,
not a reference on the point.
This fixes a memory leak introduced in 3.17's android fence rework.

This is caused by only having one channel out of the pair configured in
DT; the second channel remains uninitialised, but upon unbind, the
driver attempts to clean up both, thereby dereferencing a NULL pointer.
Avoid this by checking that the second channel is initialised.

When using the cfg80211_inform_bss[_width]() functions drivers
cannot currently indicate whether the data was received in a
beacon or probe response. Fix that by passing a new enum that
indicates such (or unknown).
For good measure, use it in ath6kl.

In the cfg80211_rx_mgmt(), parameter @gfp was used for the memory allocation.
But, memory get allocated under spin_lock_bh(), this implies atomic context.
So, one can't use GFP_KERNEL, only variants with no __GFP_WAIT. Actually, in all
occurrences GFP_ATOMIC is used (wil6210 use GFP_KERNEL by mistake),
and it should be this way or warning triggered in the memory allocation code.
Remove @gfp parameter as no actual choice exist, and use hard coded
GFP_ATOMIC for memory allocation.

initialisation
Fix two reported bugs, caused by et131x_adapter->phydev->addr being accessed
before it is initialised, by:
- letting et131x_mii_write() take a phydev address, instead of using the one
stored in adapter by default. This is so et131x_mdio_write() can use it's own
addr value.
- removing implementation of et131x_mdio_reset(), as it's not needed.
- moving a call to et131x_disable_phy_coma() in et131x_pci_setup(), which uses
phydev->addr, until after the mdiobus has been registered.

"drm_file->is_master is not synomous with having drm_file->master ==
drm_file->minor->master. This is because drm_file->master is the same
for all drm_files of the same generation and so when there is a master,
every drm_file believes itself to be the master. Confusion ensues and
things go pear shaped when one file is closed and there is no master
anymore."

All other add functions for lists have the new item as first argument
and the position where it is added as second argument. This was changed
for no good reason in this function and makes using it unnecessary
confusing.
The name was changed to hlist_add_behind() to cause unconverted code to
generate a compile error instead of using the wrong parameter order.

Currently map_vm_area() takes (struct page *** pages) as third argument,
and after mapping, it moves (*pages) to point to (*pages +
nr_mappped_pages).
It looks like this kind of increment is useless to its caller these
days. The callers don't care about the increments and actually they're
trying to avoid this by passing another copy to map_vm_area().
The caller can always guarantee all the pages can be mapped into vm_area
as specified in first argument and the caller only cares about whether
map_vm_area() fails or not.
This patch cleans up the pointer movement in map_vm_area() and updates
its callers accordingly.

The drm_file->is_master field is redundant as it's equivalent to:
drm_file->master && drm_file->master == drm_file->minor->master

Whenever we set drm_file->is_master, we also set:
drm_file->minor->master = drm_file->master;
Whenever we clear drm_file->is_master, we also call:
drm_master_put(&drm_file->minor->master);
which implicitly clears it to NULL.
2) "<="
minor->master cannot be set if it is non-NULL. Therefore, it stays as
is unless a file drops it.
If minor->master is NULL, it is only set by places that also adjust
drm_file->is_master.
Therefore, we can safely drop is_master and replace it by an inline helper
that matches:
drm_file->master && drm_file->master == drm_file->minor->master

As per Ian, at this point in time it is not worth implementing an async
command interface for diagnostic interrupts for this board. As this is
the case, this patch removes the code which supports such interrupts as it
is now unused.
This includes removing apci1564_do_read(), which was the insn_read
operation for the digital output subdevice, since all it was doing was
reading the current diagnostic interrupt type and returning it in 'data'.
This doesn't follow the comedi API and this operation can be emulated by
the comedi core anyway since the insn_bits operation follows the comedi
API. So it is safe to simply remove this function.

This board provides VCC/CC diagnostic information, and it also supports
diagnostic interrupts. However, as per Ian, these interrupts aren't very
useful and it is enough to simply provide an interface for accessing the
diagnostic status on-demand. This patch adds a 2-channel digital input
subdevice with an insn_bits handler to access this information.

The "ii_pci20kc" module is a comedi driver for Intelligent Instruments
PCI-20001C carrier board and modules. Despite the name, this is
actually an ISA board and uses 1K of ISA memory space (below 1M) for the
main board plus up to three modules. The address is set by hardware
jumpers.
When the board is attached to Comedi via the `COMEDI_DEVCONFIG` ioctl
and the driver's legacy "attach" handler, the base address is passed in.
The driver currently uses that address as-is, which is a bad idea. It
doesn't even reserve the memory region.
Fix that by sanity checking the passed in address, reserving the memory
region and ioremapping it.
Replace the current "detach" handler `comedi_legacy_detach()` with a new
handler `ii20k_detach()` which unmaps the memory and releases the
region.
