The recent commit a6b7a407 when back-ported to the out-of-tree e1000e
driver caused a compilation error on older kernels which required a
re-ordering of the #include files. This cosmetic patch syncs the two
drivers for easier maintainability.

The PCIE capability offset is saved during PCI bus walking. It will
remove an unnecessary search in the PCI configuration space if this
value is referenced instead of reacquiring it.

Doing a PHY page select can take a long time, relatively speaking. This
can cause a significant delay when updating a number of PHY registers on
the same page by unnecessarily setting the page for each PHY access. For
example when going to Sx, all the PHY wakeup registers (WUC, RAR[], MTA[],
SHRAR[], IP4AT[], IP6AT[], etc.) on 82577/8/9 need to be updated which
takes a long time which can cause issues when suspending.
This patch introduces new PHY ops function pointers to allow callers to
set the page directly and do any number of PHY accesses on that page.
This feature is currently only implemented for 82577, 82578 and 82579
PHYs for both the normally addressed registers as well as the special-
case addressing of the PHY wakeup registers on page 800. For the latter
registers, the existing function for accessing the wakeup registers has
been divided up into three- 1) enable access to the wakeup register page,
2) perform the register access and 3) disable access to the wakeup register
page. The two functions that enable/disable access to the wakeup register
page are necessarily available to the caller so that the caller can restore
the value of the Port Control (a.k.a. Wakeup Enable) register after the
wakeup register accesses are done.
All instances of writing to multiple PHY registers on the same page are
updated to use this new method and to acquire any PHY locking mechanism
before setting the page and performing the register accesses, and release
the locking mechanism afterward.
Some affiliated magic number cleanup is done as well.

Start the Tx queue when the interface is brought up in e1000e_up() but do
not schedule the queue until link is up as detected in the watchdog task
which sets netif_carrier_on.
Also flush the descriptors and clean the Tx and Rx rings before resetting
the hardware when bringing the interface down otherwise there is a small
window where the watchdog task can be triggered with netif_carrier_off
and the Tx ring not yet empty which causes an additional and unnecessary
reset.

When repeatedly cycling Sx->S0 states with the network cable unplugged,
the 82579 PHY may not initialize as expected and may require a full power
cycle to recover functionality to the device. Workaround this by testing
access of the PHY registers after resuming; if that returns unexpected
results toggle the LANPHYPC signal to power cycle the PHY.
This is implemented in the new function e1000_resume_workarounds_pchlan()
which calls another new function, e1000_toggle_lanphypc_value_ich8lan(),
which has been created to reduce code duplication (same functionality
required by a previous workaround). Also, e1000e_disable_gig_wol_ich8lan
is now e1000_suspend_workarounds_ich8lan to better reflect what it does.

After discovering that wide use of prefetch on modern CPUs
could be a net loss instead of a win, net drivers which were
relying on the implicit inclusion of prefetch.h via the list
headers showed up in the resulting cleanup fallout. Give
them an explicit include via the following $0.02 script.

Need to use it in _e1000e_disable_aspm. This routine is used for error
recovery, where the pci_bus_sem is already held, and we don't want
pci_disable_link_state to try to take it again. So add a locked variant
for use in cases like this.

Based on a patch from Stephen Hemminger <shemminger@vyatta.com>.
The new ethtool set_phys_id takes over controlling the LED for
identifying boards. This fixes the lockout during that period.
For this device lots of extra infrastructure can also be removed by
using set_phys_id.

in h/w to cleanup the test for this functionality

Based on a patch from Naga Chumbalkar <nagananda.chumbalkar@hp.com>:
If ASPM L0s needs to be disabled due to HW errata, do it prior to
"enabling" the device. This way if the kernel ever defaults its
aspm_policy to POLICY_POWERSAVE, then the e1000e driver will get a
chance to disable ASPM on the misbehaving device *prior* to calling
pci_enable_device_mem(). This will be useful in situations
where the BIOS indicates ASPM support on the server by clearing the
ACPI FADT "ASPM Controls" bit.
Note:
The kernel (2.6.38) currently uses the BIOS "default" as its aspm_policy.
However, Linux distros can diverge from that and set the default to
"powersave".
v2: o cleanup namespace pollution of e1000e_disable_aspm(),
o fix type and initialization of the new aspm_disable_flag in a few
functions, and
o redefine FLAG2_DISABLE_ASPM_L0S to the first unused bit in
adapter->flags2.

With durations less than 20ms, the jiffies or legacy timer backed msleep()
may sleep ~20ms which might not be what the caller expects. Instead, it
is recommended to use the hrtimers backed usleep_range(). For more, see
Documentation/timers/timers-howto.txt. Issues reported by checkpatch.
In addition, remove unnecessary sleep in e1000e_write_nvm_spi().

When support for 82577/82578 was added[1] in 2.6.31, PHY wakeup was in-
advertently enabled (even though it does not function properly) on ICH10
LOMs. This patch makes it so that the ICH10 LOMs use MAC wakeup instead
as was done with the initial support for those devices (i.e. 82567LM-3,
82567LF-3 and 82567V-4).

The driver was not flushing all writebacks before unloading, possibly
causing memory to be written by the hardware after the driver had
reinitialized the rings.
This adds missing functionality to flush any pending writebacks and is
called in all spots where descriptors should be completed before the driver
begins processing.

Currently the driver increments the tx_timeout counter (an error counter)
when simply resetting the part with outstanding transmit work pending.
This is an unnecessary count of an error, when all we should be doing is
just resetting the part and discarding the transmits. With this change the
only increment of tx_timeout is when the stack calls the watchdog reset
function due to a true Tx timeout.

Some Phys supported by the driver do not remain powered off across a reset
of the device when the interface is down, e.g. on 82571, but not on 82574.
This patch powers down (only when WoL is disabled) the PHY after a reset if
the interface is down and the ethtool diagnostics are not currently running.
The ethtool diagnostic function required a minor re-factor as a result, and
the e1000_[get|put]_hw_control() functions are renamed since they are no
longer static to netdev.c as they are needed by the ethtool diagnostics.
A couple minor whitespace issues were cleaned up, too.

The ICR register is clear on read and we don't care what the returned value
is when resetting the hardware so the icr variable(s) can be removed. We
should not ignore the return from e1000_lv_jumbo_workaround_ich8lan() and
from e1000_get_phy_id_82571() (dump a debug message when it fails and when
an unknown Phy id is returned).

Some drivers are using skb_transport_offset(skb) instead of skb->csum_start
for NETIF_F_HW_CSUM offload. This does not matter now, but if someone
implements checksumming of encapsulated packets then this will break silently.
TSO output paths are left as they are, since they are for IP+TCP only
(might be worth converting though).

flush_scheduled_work() is on its way out. This patch contains simple
conversions to replace flush_scheduled_work() usage with direct
cancels and flushes.
Directly cancel the used works on driver detach and flush them in
other cases.
The conversions are mostly straight forward and the only dangers are,
* Forgetting to cancel/flush one or more used works.
* Cancelling when a work should be flushed (ie. the work must be
executed once scheduled whether the driver is detaching or not).
I've gone over the changes multiple times but it would be much
appreciated if you can review with the above points in mind.

Provide support to e1000e for displaying the new format of the PBA found
in the EEPROM. The unique PBA identifier is no longer restricted to
hexadecimal numbers and must now be read and displayed as a string.

The kernel goes BUG() at the time 'ethtool -p eth0 3' comes
back, which is due to adapter->led_blink_task initialized
several times. At the time it is still running this results
in a corrupted task_list of the associated workqueue.
The fix is to move the workqueue initialization to the
probe function instead.

With enabling CONFIG_PCI_MSI, e1000e could work in MSI/MSI-X IRQ mode,
and netpoll controller didn't deal with those IRQ modes on e1000e.
This patch add the handling MSI/MSI-X IRQ modes to netpoll controller,
so that netconsole could work with those IRQ modes.
