Rename the ia64 documentation files to ReST, add an
index for them and adjust in order to produce a nice html
output via the Sphinx build system.
There are two upper case file names. Rename them to
lower case, as we're working to avoid upper case file
names at Documentation.
At its new index.rst, let's add a :orphan: while this is not linked to
the main index.rst file, in order to avoid build warnings.

Most places use pwd and rely on $PATH lookup. Moving the remaining
absolute path /bin/pwd users over for consistency.
Also, a reason for doing /bin/pwd -> pwd instead of the other way around
is because I believe build systems should make little assumptions on
host filesystem layout. Case in point, we do this kind of patching
already in NixOS.

Remove ia64 from Makefile to move the test to selftests.
Update ia64 Makefile to work under selftests. ia64 will not be run as part
of selftests suite and will not be included in install targets. They can be
built separately for now.
The original Makefile built this test on all archirectures and this update
doesn't change that.

The general documentation we have for pv_ops is currenty present
on the IA64 docs, but since this documentation covers IA64 xen
enablement and IA64 Xen support got ripped out a while ago
through commit d52eefb47 present since v3.14-rc1 lets just
simplify, generalize and move the pv_ops documentation to a
shared place.

I got a "sched_setaffinity:: Invalid argument" error when using
err_injection_tool to inject error on a system with over 32 cpus.
Error information when injecting an error on a system with over 32 cpus:

This because there is overflow when calculating the cpumask: the
type of (1<<k) is int, while mask[j] is unsigned long. When k > 31,
(1<<k) is truncated to ZERO, resulting in a unexpected cpumask.

Below you will find an updated version from the original series bunching all patches into one big patch
updating broken web addresses that are located in Documentation/*
Some of the addresses date as far far back as 1995 etc... so searching became a bit difficult,
the best way to deal with these is to use web.archive.org to locate these addresses that are outdated.
Now there are also some addresses pointing to .spec files some are located, but some(after searching
on the companies site)where still no where to be found. In this case I just changed the address
to the company site this way the users can contact the company and they can locate them for the users.

Currently source files in the Documentation/ sub-dir can easily bit-rot
since they are not generally buildable, either because they are hidden in
text files or because there are no Makefile rules for them. This needs to
be fixed so that the source files remain usable and good examples of code
instead of bad examples.
Add the ability to build source files that are in the Documentation/ dir.
Add to Kconfig as "BUILD_DOCSRC" config symbol.
Use "CONFIG_BUILD_DOCSRC=1 make ..." to build objects from the
Documentation/ sources. Or enable BUILD_DOCSRC in the *config system.
However, this symbol depends on HEADERS_CHECK since the header files need
to be installed (for userspace builds).
Built (using cross-tools) for x86-64, i386, alpha, ia64, sparc32,
sparc64, powerpc, sh, m68k, & mips.

Example memory map (HP rx7640 with 'default' acpiconfig setting, VGA disabled):
0x00000000 - 0x3FFFBFFF supports only WB (cacheable) access
If a user attempts to perform an MMIO mmap (using the PCIIOC_MMAP_IS_MEM ioctl)
to PCI config space (like mmap'ing and accessing memory at 0xA0000),
we will MCA because the kernel will attempt to use a mapping with the UC
attribute.
So check the memory attribute in kern_mmap and the EFI memmap. If WC is
requested, and WC or UC access is supported for the region, allow it.
Otherwise, use the same attribute the kernel uses.
Updates documentation and test cases as well.

This path only exists for CONFIG_NUMA=y systems. Better to use
/sys/devices/system/cpu/cpu%d/err_inject/ which is available on all
systems.

This patch contains a documention and sample application. Since the sample
application has ~1000 lines of code, it might not be suitable in a kernel
documention in kenrel tree. If you think this is not good place to hold
the sample application, please let me know and I'm open to other choices
e.g. sourceforge etc.

We removed 8250_acpi in 2.6.17. If we don't have PNPACPI turned on, we
won't find any ACPI serial devices, so mention this requirement in the
troubleshooting part of the documentation.
CONFIG_PNPACPI is already turned on in all the relevant defconfigs.

This closes a couple holes in our attribute aliasing avoidance scheme:
- The current kernel fails mmaps of some /dev/mem MMIO regions because
they don't appear in the EFI memory map. This keeps X from working
on the Intel Tiger box.
- The current kernel allows UC mmap of the 0-1MB region of
/sys/.../legacy_mem even when the chipset doesn't support UC
access. This causes an MCA when starting X on HP rx7620 and rx8620
boxes in the default configuration.
There's more detail in the Documentation/ia64/aliasing.txt file this
adds, but the general idea is that if a region might be covered by
a granule-sized kernel identity mapping, any access via /dev/mem or
mmap must use the same attribute as the identity mapping.
Otherwise, we fall back to using an attribute that is supported
according to the EFI memory map, or to using UC if the EFI memory
map doesn't mention the region.

Initial git repository build. I'm not bothering with the full history,
even though we have it. We can create a separate "historical" git
archive of that later if we want to, and in the meantime it's about
3.2GB when imported into git - space that would just make the early
git days unnecessarily complicated, when we don't have a lot of good
infrastructure for it.
Let it rip!
