Pull AFS fixes from David Howells:
"Two fix patches for the AFS filesystem:
- Fix the refcounting on permit caching.
- AFS inode (afs_vnode) fields need resetting after allocation
because they're only initialised when slab pages are obtained from
the page allocator"
