The Fujitsu FRV kernel port has been around for a long time, but has not
seen regular updates in several years and instead was marked 'Orphaned'
in 2016 by long-time maintainer David Howells.
The SoC product line apparently is apparently still around in the form
of the Socionext Milbeaut image processor, but this one no longer uses
the FRV CPU cores.
This removes all FRV specific files from the kernel.

The previous patch renamed several files that are cross-referenced
along the Kernel documentation. Adjust the links to point to
the right places.

There are numerous broken references to Documentation files (in other
Documentation files, in comments, etc.). These broken references are
caused by typo's in the references, and by renames or removals of the
Documentation files. Some broken references are simply odd.
Fix these broken references, sometimes by dropping the irrelevant text
they were part of.

Update kernel-parameters.txt to point users to the authoritative comment
for name_to_dev_t. In addition, updates other places where some
name_to_dev_t behavior was discussed. All other references to root=
appear to be for explicit sample usage or just side comments when
discussing other kernel parameters.

My first guess for "fujitsu" was it might be related to the
fujitsu-laptop.c driver...
Move the frv directory one level up since frv is the name of the
architecture in the Linux kernel.
