xen: mask extended topology info in cpuid

xen: set up mmu_ops before trying to set any ptes

x86-64 assumes NX is available by default, so we need to explicitly check for it before using NX. Some first-generation Intel x86-64 processors didn't support NX, and even recent systems allow it to be disabled in BIOS. x86: Move get/set_wallclock to x86_platform_ops

get/set_wallclock() have already a set of platform dependent implementations (default, EFI, paravirt). MRST will add another variant.
